Hello, my name is Chelsey Forsyth. After 23 years in front-line healthcare, as well as extensive training and experience in dementia and palliative care, I decided to return to college and further my education as an SSW. I have built a multi-disciplinary team of social service workers, personal support workers, and gerontology experts who offer a professional, supportive, and person-centered transition for seniors and their families. From caregiver burnout to simply navigating applications/forms and community supports or home placement. We are here to support you and your loved one through the stressful and emotional challenges that come with moving from home to a facility and/or a facility to hospice. We partner with community agencies, supportive programs, government-funded supports, and retirement/LTC homes. Downsizing, decision making, and exploring ongoing programming/activities geared to an individual’s specific needs and preferences. Helping you navigate resources and discovering what fits best to your lifestyle. We also provide exceptional temporary PSW care in the interim until you are completely set up and settled in. All of our PSW’s are highly trained, government registered, insured and have a minimum of 10 years experience. We also offer mental/emotional health support throughout your transition process and post-move follow ups and check-ins.
